A: On the left: frontal view of a cock-headed Anguipes in armour, head in profile to right. Right hand raised high and holding a whip, left hand holding a round shield, with a unique, double rim. Incisions projecting to the sides at the bottom of the pteryges are also noteworthy. On the right, frontal view of the bandaged mummy of Osiris, in wig and crown, holding flails in both hands, arms bent in elbow. Between the two figures, a six-pointed star. Inscription around, beginning at top left: φιλοσεραπιναγαθωνα (the sigma is a subsequent correction) → φιλοσέραπιν Ἀγάθωνα. 'Agathon, worshiper of Sarapis' (acc.).
B: Inscription in the middle: αβρασαξ → Ἀβρασάξ. Around, framing it from four sides, four more voces magicae. Top left, in two lines: αβλανα|θαναλβα → Ἀβλαναθαναλβα, palindrome. Top right, in two lines: ηλια|μβρω → ηλιαμβρω. Bottom left, in two lines: σεμε|σιλαμ, and a small, six-pointed star below it. Bottom right, in two lines: δαμνα|μενευ → Δαμναμενεύ.
Oval, slightly convex on both sides. Intact, with bubble-like chippings.
