A: On the left, the mummy of Osiris wearing the atef-crown with ram's horns and uraei, holding flails in both hands. On the right, an ithyphallic baboon, standing to left, with both paws raised in greeting, head crowned with the full moon or the solar disk. Above, between the two figures, a lying crescent and an eight-pointed star.
B: Inscription in three lines: ιαωσ|αβαωθ|αβασαζ → Ἰάω, Σαβαώθ, αβασαζ (variant of Ἀβρασάξ).
