Description: Upright rectangular, both faces flat. Blue lapis lazuli. Chipped at the edge, half is missing.

A: Multiple-headed figure (probably with the head of a lion and a ram) with human body, crowned with a lotus flower, wearing a necklace or an armour, holding the sa-sign with both hands, standing on a Ouroboros enclosing the following scene (from left to right): crocodile with the head of a falcon, bending woman, ithyphallic dog standing on his rear legs. Under the Ouroboros: a mummy. At the two sides of the two-headed figure: σερ|φουθ|μου|ισρω → Σερφουθ Μουι Σρω.
B: Greek inscription in eight lines: ιεωαιη|αιωουευ|αιηαηιωη|ωαιηουευ|ωαιεευ[…]|οωνεηο[…]|υιαωαι|τιτοθη → vocales and variant of Τιτώης.
