Description: Side A upright oval, side B transverse oval, both faces flat. Shape 8–9. Green prase. Chipped around the edges.

A: Lion-headed Chnoubis, head to left and crowned with rays, four of which remain visible. There is an inscription surrounding the figure. In front of him, horizontally: σια, in front of him, in two vertical lines: χνου|μισ. Behind the figure: ανοχ. Below: σεμ[- - -]αμψ. Above:  γι[- - - -]|κτα [- - -]φι[- -] → Ἀνοχ Χνουμίς. 'I am Chnoumis.', variant of σεμεσειλαμ, γι[γαντορή]κτα, [βαρο]φί[τα].  'Giant-slayer, crusher of snakes.'
B: Three lines of three characteres: six-pointed stars, Chnoubis-signs, Z-signs.
