A: In the center: four-winged mummy with two animal protomes as heads (jackal-heads?) standing on a base constructed of two groundlines. Below the wings, on the left: the mummy of Osiris, holding a flail in each hand; on the right: a female figure standing head to left, wearing a long garment and headband, left hand lowered beside the body, right hand raised to mouth. Illegible inscription in three lines around the edge at the top.
B: Inscription in eight lines: ιαωσ|αβαω[..]|βαωαβρ|ασαξμι|χαηλσου|μαρταα|δωναι|ιεω → voces including Ἰάω, Σαβαώ, Ἀβρασάξ, Μιχαήλ, σουμαρτα, Ἀδωναΐ, variant of Ἰάω.
