A: Pantheistic figure standing to front, feet to right. Human body with three animal heads: falcon (in the middle, to right), jackal (on the left, to left), and ibis (on the right, to right), all topped with stylized crowns. Body wrapped in a tunic-like mummy bandage from neck to knees, hands held together on the chest. One pair of wings behind the shoulders, another, smaller pair of wings behind the thighs.
B: Inscription in six lines: βιχω|βιχωβε|υβευχωβ|ιχωβιβε|υσουμα||ρτα → voces including σουμαρτα.
